Population Health Management Summit Starts Next Week

​Proof of Vaccination Required to Attend

Online registration has ended for the upcoming Population Health Management Summit, but you can still register in-person at the JW Marriott Washington, D.C. and attend December 6-7.

With excellent education sessions, exciting speakers, and unique networking opportunities, this summit will help steer you towards a more successful and sustainable future. Key topics include:

  • ​Special Needs Plans (I-SNPs, C-SNPs, D-SNPs)
  • Provider networks in value-based care
  • Innovations such as Nursing Home Without Walls, Hospital at Home
  • Intentional partnerships in assisted living and value-based care
  • Preparing for rapid change
  • Opportunities and challenges in population health and value-based care

Please be aware that you are required to submit proof of full vaccination in order to attend the PHM Summit. To be considered fully vaccinated, you must be two weeks past your second dose in a two-dose series (Pfizer or Moderna) OR two weeks past a single-dose vaccine (Johnson & Johnson/Janssen). 

An image of your vaccination card (which includes name of person vaccinated, type of vaccine provided, and date last dose was administered) is considered proof of vaccination and should be uploaded via our secure link prior to arrival on site. Proof of vaccination documentation will be stored in a HIPAA/PHI compliant online dashboard when collected. Additionally, you’ll be asked to sign a COVID-19 Health & Safety liability waiver.
