The LTC Data Cooperative is a powerhouse for data analytics, serving both facility needs and offering a critical resource for long term care research. By assembling electronic health record (EHR) data from LTC organizations, the LTC Data Cooperative gives participants access to data reports specific to the facilities in their organization. Additionally, the available data supports academic research on the effectiveness of treatments and care delivery models on resident outcomes, which in turn produces findings that SNFs and AL communities can use to improve care. By participating in The LTC Data Cooperative, participants will be contributing to the larger LTC and PAC community's efforts.
While the Data Cooperative provides EHR data for reports to providers and for research purposes, LTC Trend Tracker offers reports to providers based on data collected from the Minimum Data Set, Care Compare, and CASPER, just to name a few. LTC Trend Tracker adds another layer of insight, providing trending data, benchmarks to different facility peers, and goal-setting tools that assist providers in gathering more information on a wide variety of metrics. LTC Trend Tracker provides facilities with the opportunity to see how they perform against others in areas such as quality, staffing, survey, and financial metrics by running reports and subscribing to facility-specific publications. LTC Trend Tracker also provides interactive products within the system, such as the Five-Star Quality Rating and Value-Based Purchasing tools where users can simulate best- and worst-case scenarios to use in their quality-improvement effort plans.
