The National Center for Assisted Living (NCAL) has released
The Value of Assisted Living: A Pathway to Value Based Care, a new checklist to help assisted living (AL) providers understand, assess, and strengthen capabilities that support participation in value-based care (VBC). This checklist was informed by AL providers participating in the NCAL Assisted Living Population Health Management (PHM) Workgroup.
The tool includes two resources:
- Capabilities Checklists: Help providers assess their capabilities across early, intermediate, and advanced stages of VBC readiness.
- Checklist Companion Guide: Defines and explains key terms used in the checklists, describes why each capability matters, and provides examples.
Providers can use the checklist to:
- Identify capabilities already in place;
- Recognize where additional information and partnerships may be helpful;
- Inform discussions with primary care providers, physician groups, health plans, ACOs, and other potential partners; and
- Identify questions to explore before deciding whether or how to participate in value-based care.
The checklists are a starting point for learning and planning, not as a score or set of requirements. Providers are not expected to have every capability in place or to develop every capability internally. Many capabilities may be supported through partnerships with primary care and other clinical providers, health plans, accountable care organizations, health systems, pharmacies, and technology organizations.
