Registration is now open for AHCA/NCAL’s 6th Annual Population Health Management (PHM) Summit, taking place April 15-16, 2026, in Baltimore, MD.
We are excited that this year’s Summit features three targeted education tracks, including one for assisted living (AL), and two high-impact keynote sessions from Abe Sutton, Director of the CMS Innovation Center (CMMI, invited), and Mark Atalla, Deputy Assistant Secretary for Technology Policy/Deputy National Coordinator for Policy (HHS).
The Assisted Living track is designed for AL leaders seeking to understand and engage in value-based care and population health management. In addition, AL attendees have access to sessions across the Summit's other tracks. Here are the sessions most relevant to your work:

- Long Term Care in ACO Models: Opportunities in LEAD and Beyond

- From Model to Match: Building Successful Value-Based Care Partnerships
Learn how to choose the right value-based care model and the right partners. This session covers how to assess financial capability, clinical and operational readiness, cultural alignment, and technology capability—plus, common red flags and strategies for building accountable, high-performing partnerships.
